Once the application is complete, please submit it to our accounting office for processing. There is a non-refundable application fee of $15 per student, with a maximum of $25 per family. Applicants are required to attach a copy of the student's most current report card, a copy of their current transcript, and a copy of their most recent test scores (SAT9, SAT10 or CTBS).

All athletes are required to have their physical completed prior to the first practice for their sport. We recommend that you schedule your son or daughters' physical in June or early July. By doing so, you will avoid the rush and your student will never have to worry about missing practice during the school year. Physicals are good for one calendar year from the date they are completed. These physicals should be completed by your family physician or pediatrician. Students without a current physical will not be allowed to practice or play.

This file consists of six pages, the Athletic Participation checklist (Page 1); the Agreement for Athletic Participation (Page 2) to be signed by the athlete and the parent; the Athletic Consent/Insurance form (Page 3) to be completed and signed by the parent; the History Form (Page 4) to be completed and signed by the athlete and parent; the Physical Exam Form (Page 5) and the Clearance Form (Page 6) to be completed and signed by the student's physician.
